A Parent-to-Parent Guide for Families Facing Congenital Heart Disease

When a doctor tells you that your child has congenital heart disease (CHD)—such as Tetralogy of Fallot (TOF) or Ventricular Septal Defect (VSD)—it can feel overwhelming.

Many parents immediately start asking the same questions:

How will we afford the surgery?Where do we go?What should we do first?

If you are reading this, please know something important:

You are not alone.

Many parents in the Let It Echo community have walked this same journey. And the encouraging news is that life-saving heart surgery is possible in the Philippines, and programs like PhilHealth Z Benefits can help cover the majority of hospital costs.

This guide was created by parents who have experienced the process, to help other families navigate the steps with more clarity, confidence, and hope.

What is PhilHealth Z Benefits?

The PhilHealth Z Benefits Package is a program designed to help Filipino families manage the cost of serious medical conditions, including open-heart surgery for congenital heart disease.

For children approved under the program, PhilHealth typically covers most hospital expenses, such as:

  • Hospital admission and room

  • Operating room fees

  • Surgical equipment

  • ICU stay

  • Medicines used during hospitalization

  • Laboratory tests during admission

Parents may still need to prepare for Doctors’ Professional Fees, which are commonly around:

₱200,000 in cashor₱400,000 worth of Guarantee Letters (GLs).

Many families are able to gather this support through government assistance, charitable organizations, and community fundraising.

What is a Guarantee Letter?

A Guarantee Letter (GL) is financial assistance provided by public officials or organizations that can help cover doctors’ professional fees.

Families commonly request GLs from:

  • DSWD Department of Social Welfare Development

  • DOH Department of Health

  • Senators

  • Members of Congress

  • Party-list representatives

  • Local government officials

  • Charitable foundations

  • Community organizations

Groups such as Let It Echo, civic clubs, and community networks can sometimes help families connect with possible sponsors or guidance on where to request assistance.

Many parents combine Guarantee Letters with fundraising activities to reach the needed amount.

The General Process

While every case is slightly different, many families follow these general steps when applying for PhilHealth Z Benefits for congenital heart surgery.

1. Schedule a Cardiology Consultation

The first step is to consult with a pediatric cardiologist who can evaluate your child’s condition.

Your doctor may refer you to a hospital that performs congenital heart surgery and participates in the PhilHealth Z Benefits program.

Examples of institutions that provide specialized cardiac care include hospitals such as Philippine Heart Center and other accredited cardiac centers in the country. These are only examples—families should consult their doctor to determine the best option for their child.

  • During the consultation, the cardiologist may:

  • Review your child’s diagnosis

  • Evaluate previous test results

  • Request additional tests

  • Determine if surgery is recommended

2. Prepare the Required Documents

To make the process smoother, it is helpful to prepare some documents before traveling for consultations or hospital evaluation.

Documents to Prepare in Your Local Area

  • PhilHealth ID of the parent member

  • MDR (Member Data Record)

  • CF1 Form (PhilHealth Claim Form)

  • Updated PhilHealth contributions

  • MMR Vaccine Certificate - Often recorded in the child’s health card can be certified by your local health center

  • Chickenpox Vaccine Certificate - Usually provided by a pediatrician or clinic

These documents help confirm PhilHealth eligibility and vaccination records.

Requirements That May Be Completed at the Hospital

Some medical requirements are typically completed at the hospital where the surgery will be performed, such as:

  • Laboratory tests

  • Dental clearance

Dental clearance is often required to ensure there are no infections that could affect the heart during surgery.

3. Request a Surgery Quotation

After evaluation, the cardiologist or hospital may provide a surgery quotation.

This document is often needed when applying for PhilHealth Z Benefits coverage.

4. Apply for the PhilHealth Z Benefits Package

The hospital’s PhilHealth office will usually guide families through the application for the Z Benefits package.

This step includes submitting the required medical documents and PhilHealth records.

5. Wait for PhilHealth Approval

Approval may take several weeks, depending on the case and documentation.

While waiting for approval, many families begin preparing support for professional fees and related expenses.

6. Seek Support and Prepare Financial Assistance

Parents often request Guarantee Letters from government offices or organizations during this period.

Some families also organize community fundraising efforts to help cover:

  • Doctors’ professional fees

  • Transportation to the hospital

  • Temporary accommodation

  • Food and daily expenses during hospitalization

Simple Fundraising Ideas

Even small efforts can help make a big difference.

Many parents have organized activities such as:

T-Shirt for a Cause

Example:

Selling shirts for ₱500 each and sharing the campaign through social media.

Funds raised can help support the family during the hospital stay.

Communities often come together when they understand a child is fighting for a healthy heart.
