My CHD kid needs help in surgery. How do I apply?

We have good news! The Pediatric Cardiac Mission is still ongoing. Our strong partnership continues with Gift of Life International and Rotary Club of Cebu. We have better and more sustainable agreement with our partner hospital De Los Santos Medical Center. This means we can help more kids! :)
We continue to help indigent Filipino kids who has Congenital Heart Defect (CHD) categorized as simple case and can be fixed through interventional catheterization procedure. For non-simple cases, scroll down to the end of this post for more info.
Here is the selection process:
1. Apply online through message to Let It Echo FB page. Fill in online form [link]. 2. Provide patient information.

3. Provide requirements for evaluation.
DSWD social studies report
Barangay certificate of indigency
PhilHealth MDR (has to be active and patient should be dependent)
Birth certificate of patient
Picture - full face, full body, hands and fingers
Valid IDs na government-issued of parents
Medical documents (2d echo, 2d echo video clips)
4. Review will be done by medical team. Review will be done by us on social standing.
5. If the case is doable, we'll proceed with a schedule. Only hospital bill (surgery cost + doc fee) is part of the sponsorship. The hospital is Delos Santos Medical Center at Quezon. The parents will need to shoulder the cost for their logistics, accommodation, swab tests, and labs. Max number of days stay is 10 days in Manila. Only 2 days in the hospital.
